How do I access Institutional Review Board ( IRB ) forms related to my dissertation?

I am looking for [ Form 1 or Form 2 or Form 3 or Form 12...].
These numbered forms, such as the one signed by the dissertation committee at the oral defense, are not available from the library. Usually you can find them on the student portal in the IRB section (https://alliantintluni.sharepoint.com/sites/IRBStudent), in your program's dissertation manual, or by contacting your program's academic coordinator.
